All the forms listed below show be completed and submitted (these forms will be made accessible upon payment of your: Registration fee of JA$2500 and Program fee deposit of US$500)
- SWAT Clarity Contract
- School Status Letter
- Upload Resume (Excluding Self-Placed Applicants)
- Two Passport size photos (photos will be taken in office and is already covered in your program fee)
- School ID
- Passport (Please check passport to ensure it does not expire before March 2017)
- Transcript or Progress Report (Must be stamped by School)
